Type 2 Diabetes


Prevention, Screening, and Intervention Program


Type 2 Diabetes Prevention, Screening, and Intervention Program's goals are to (1) increase early identification of those at high-risk for developing diabetes and provide education and promote behavioral changes that prevent its development; (2) increase early diagnosis of those with diabetes and provide education and support to help them manage their disease; and (3) educate providers to promote quality diabetes care in the community. Community health promoters provide outreach education about type 2 diabetes at local churches, schools, and community-based organizations and encourage class participants to complete the American Diabetes Association's Diabetes Risk Test. Participants scoring 10 or more points are at a greater risk for for having diabetes and are referred to a health care provider to determine if they have diabetes. Participants found not to have diabetes are invited to participate in Primary Diabetes Prevention Services that includes 5 weekly workshops promoting healthy eating and increased physical activity.

Newly diagnosed type 2 diabetics will be invited to participate in Secondary Prevention Services that include:

Diabetes Empowerment Education Program (DEEP): 8 weekly workshops designed to help patients understand and manage their diabetes.

Take Charge! With the Diabetes Health Record: this class teaches those with diabetes basic care guidelines, the importance of managing diabetes and how to access support, using the Diabetes Health Record, and how to reduce/delay complications through controlling blood sugars and regular health care visits.

Chronic Disease Self-Management Program (CDSMP): This 6 week intervention is based on self-efficacy theory and emphasizes problem-solving, decision making and confidence building. K.R. Lorig et al of the Stanford Patient Education Research Center designed the program. Two bilingual, indigenous lay leaders lead the 6-week small group intervention using a highly structured manual. The sessions are highly participatory. Mutual support and success builds patients' confidence in their ability to manage their health and maintain active and fulfilling lives.

Nutritional Counseling: Program dieticians provide nutritional counseling based on the national Diabetes Education Program's Recipe and Meal Planner Guide: Eating Healthy and Staying Fit to Control and Manage Diabetes. Classes include cooking demonstrations and meal planning.

Additional educational interventions include referrals to dental hygiene class given by Community Dental Partnership community health promoter, referrals to dental care through the UniHealth Community Dental Partnership Program, and referrals to smoking cessation classes, as needed.
