Before you are admitted to California Hospital Medical Center or receive outpatient care at the hospital, you will need to complete the registration process.
Inpatient/Outpatient Admissions
If you are being admitted to the hospital, please come to the Admitting Department. Once you have completed the registration process, you will be escorted to your hospital room.
What to Bring:
- Photo
- ID Insurance card(s)
- Any forms, paperwork, physician orders, pre-op labs, or x-ray results from your doctor's office
- List of current medications
- Your healthcare advanced directive form if you have completed one. (If you do not have a healthcare advanced directive but would like one, the Admitting Department can provide you with information)

Monday through Friday
6:00 a.m. - 6:30 p.m.
Saturdays
7:00 a.m. - 6:30 p.m.
After business hours, and on Sundays and holidays, please go to Emergency Room Admitting.
